Step by step

What you need to pre-enroll

All of the requirements and documentation needed to pre-enroll in MOAO.

Parent & student information

Provide basic info including name, contact info, and your student's date of birth.

Ohio residency

Provide proof of your Ohio residency with two of the following documents: utility bill, mortgage or lease, bank statement or pay stub, property tax bill, Ohio driver's license or state ID, voter registration card, or notarized affidavit of residency from the homeowner.

Student identity

Provide proof of your student's identity with one of the following documents: birth certificate (preferred), passport, parent affidavit, or previous school records.

Immunization record or exemption

Ohio state law requires that you provide your student's immunization records or a medical or religious exemption form. You can complete your enrollment without any immunization or exemption documentation, but you will be required to provide it within 14 days of your student's start date.

Academic records

If available, provide any report cards, transcripts, homeschool documentation, IEP, or 504 plan to ensure that we can get your student started on the right track.
